Patient Forms
If you are a new patient to our office, please visit our patient forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
You can pay your bill directly by calling us at (603) 524 - 8159 to use a credit/debit card over the phone. Alternatively, you can put your credit card information on your invoice and mail it in to us.
For your convenience, we accept cash, personal checks, debit cards, CareCredit and most major credit cards. Payment is expected at the time services are performed. Feel free to contact our office if your treatment plan requires more comprehensive dental work and you want to discuss financial arrangements.

An Important Note About Dental Insurance
Dental Insurance is a great benefit. However, it is important to remember that your employer chose your company’s plan based on their financial ability and not on the employees individual dental needs.
Sometimes we see that our patients are frustrated when their insurance denies or only partially covers a recommended treatment. We completely understand this frustration! We understand that your first instinct may be to postpone necessary treatment or even sacrifice your oral health because of this.
Dr. Johnson and staff pride ourselves on putting patient care first! Our treatment plans are created with only your oral health in mind, not what your insurance coverage is.
